HOW SHOULD I PREPARE FOR THIS TYPE OF APPOINTMENT?

• Feel free to bring a drink and snacks as you will spend several hours in the office.

• Bring a sweater in case you get cold in the office.

• If you need glasses and/or hearing aids, make sure to bring them with you.

• Bring any available medical records.

• Complete any intake paperwork ahead of time. You can either email, fax, or bring it with you on the day of your appointment.

• If you have a friend, spouse, child, or anyone else who knows you well, feel free to ask them to join you for the clinical interview, as obtaining collateral information is very helpful. Keep in mind, they will not be allowed in the office during the testing part.
